If you are ready to begin your career today, these are the following
steps to begin your application at Basali' Dermal Institute.
- Submit completed student
application one of two ways:
- Submit a letter of intent
describing your career goals
- Submit a letter of
recommendation from a business professional
- Submit a letter from a health
care professional stating you are free from communicable diseases and
in good health.
- Prompt your high school to
submit a copy of your transcripts or GED
- Submit a non-refundable $50
registration fee
- Complete a tour with an
admissions representative
Once these materials have been received and you have taken a tour of
the school, you will receive a written notice of acceptance or
rejection. Upon acceptance into the Basali' Dermal Institute program,
the following will need to be completed and returned to the Admissions
Department to reserve your space in the program.
- Sign an enrollment contract
- Sign and submit a payment

Cancellation
Notice
If a student decides to cancel her/his enrollment, she/he may do so
only by a formal written notice given to administration. A student may
hand deliver or mail the notice with a return receipt requested to the
address listed on their enrollment contract
Constructive
notice policy: A student will be considered withdrawn by
the school if she/he fails to attend class, utilizes instructional
facilities or submit lessons without providing, prior to or during that
period, an explanation to the school regarding the inactivity after
three days and no notice is given. The postmark date on the written
notification or the date the student gives said notice to
administration is the official withdrawal date of the student.
Breach
of Contract
Basali' Dermal Institute may terminate a student from the program'r
cancel enrollment if she/he breaches or violates any part of the
enrollment contract or school policies. Students would receive written
notice of cancellation from the school. A studentís
enrollment can be cancelled only by written notice of cancellation from
one party to another. A cancellation notice will either be sent by
registered or certified mail to the address last recorded in the
student's file or hand delivered to the student. |
